In my opinion, there probably isn't a fitting image for this article at the moment. Keep in mind, Simple English Wikipedia only uses images that are already hosted on wikimedia commons. Also, it appears some of your changes to this page have been reverted. In very simple terms, gender disphoria seems to be a man "feeling he should be a woman" (or a woman feeling "she should be a man"); so we have two opions, showing a well-known person with that problem, ot sjhowing a woman, with very masculine featutres (or a man that looks very feminine).
